Whole-Body Hyperthermia with the Iratherm® 1000 (wIRA)in Fibromyalgia

Whole-Body Hyperthermia for Fibromyalgia

A science-backed therapy for lasting pain relief

Fibromyalgia is one of the most challenging chronic pain conditions to treat — affecting sleep, mood, and quality of life alongside widespread musculoskeletal pain. At Artisan Clinic, we offer Whole-Body Hyperthermia (WBH) using the Iratherm® 1000, a medically registered device that uses water-filtered infrared-A (wIRA) radiation to gently raise your core body temperature into a controlled, fever-like range.

This is not a sauna. It is a precision medical therapy — and there is a growing body of peer-reviewed evidence behind it.

How It Works

The Iratherm® 1000 uses water-filtered infrared-A light to warm the body from the outside in, bringing your core temperature to approximately 38.5–38.7°C — similar to a mild natural fever. This therapeutic warmth is held for a short plateau period, triggering your body's natural pain-dampening responses.

For fibromyalgia, we use the mild hyperthermia protocol — not the higher-dose oncological range. Sessions are comfortable, monitored throughout, and typically last 60 minutes.

What the Research Shows

Multiple peer-reviewed studies, conducted over nearly three decades across several German medical centres, have consistently found that mild wIRA whole-body hyperthermia reduces fibromyalgia pain and is well tolerated.

The strongest evidence to date:

Langhorst et al., Journal of Clinical Medicine 2023 — the only sham-controlled randomised trial of this therapy in fibromyalgia. In 41 patients, six WBH sessions over three weeks produced:

  • Significantly lower pain scores at 4 weeks (p = 0.015)
  • Benefits that persisted at 7 months (p = 0.002)
  • No withdrawals due to side effects

Brockow et al., Clinical Journal of Pain 2007 — a randomised controlled trial in 139 inpatients found that adding WBH to a standard rehabilitation programme produced a moderate additional reduction in pain (Cohen's d ≈ 0.60) compared to rehabilitation alone.

Walz et al., Schmerz 2013 — a three-arm controlled trial found pain benefits persisting to 6 months, and notably found that once-weekly treatment outperformed twice-weekly — suggesting a lower-frequency schedule may be optimal.

The honest picture: the evidence is promising and consistently positive, the therapy is well tolerated, and the best results come when WBH is used alongside your existing care — not as a replacement for it.

What a Course of Treatment Looks Like

Sessions: 6 outpatient sessions

Schedule: Over 3 weeks (at least one rest day between sessions)

Duration: ~60 minutes per session

Target temperature: 38.5–38.7°C core

Monitoring: Core temperature, pulse and oxygen levels throughout

Hydration: You will be asked to stay well hydrated

Is It Right for You?

WBH is generally well tolerated. However, it is not suitable for everyone. We will always complete a thorough screening before recommending this therapy.

This therapy may not be appropriate if you have:

  • A current febrile illness or core temperature above 37.5°C
  • Significant cardiovascular disease (e.g. heart failure)
  • Severe comorbidities (somatic or psychiatric)
  • Pregnancy

Please inform us of all medications you take, particularly those affecting fluid balance or thermoregulation.

Device & Regulatory Status

The Iratherm® 1000M is a registered medical device in the EU, Singapore, and Malaysia.

Jurisdiction | Status

European Union | CE-marked, Class IIa

Singapore (HSA/SMDR) | Registered Class B — Reg. no. DE0506482

Malaysia (MDA) | Registered Class B — Reg. no. GB6786221-55575

The device is registered for whole-body hyperthermia and relief of muscle fatigue, stiffness and pain. As with most physical therapies, regulatory approvals are for the device and its method — not a fibromyalgia-specific disease indication.

Frequently Asked Questions

Is this the same as a sauna or infrared sauna?
No. The Iratherm® 1000 uses a specific, medically calibrated water-filtered infrared-A wavelength with precise temperature monitoring. It is a Class B registered medical device, distinct from consumer wellness products.

How soon will I feel a difference?
Research suggests some patients notice improvement during or shortly after the first sessions, while for others benefit builds over the course of treatment and beyond. One study found the duration of time held at target temperature may predict how long-lasting the relief is.

Can I continue my other treatments?
Yes — and we encourage it. The evidence supports WBH as a complement to your existing care, not a replacement.

Are there side effects?
Side effects are mostly short-lived physiological responses to warmth — similar to what you might experience after gentle exercise. In the only sham-controlled clinical trial, no patients withdrew due to side effects.
